Chronic kidney disease needs to be monitored at least annually, to identify those at high risk of rapidly declining kidney function Chronic kidney disease is common, but only a very small percentage of people who have it require dialysis or transplantation There is much that can be done to slow the progression of kidney disease in partnership with the patient
